*Total payment is due at the start of each membership year. Average Preventive Value: $810.00 per person. $25.00 one-time application fee per person. $50.00 maximum application fee per family. Prices effective January 1, 2023.

What’s Included?
- 2 cleanings per plan year
- 2 periodic exams per plan year
- 1 problem focused exam per year
- 4 bitewing x-ray films per plan year
- 2 periapical x-ray films per plan year
- 1 full mouth x-ray (usually taken every 3-5 years)
- 2 fluoride treatments per plan year for children up to age 15
Silver Plan vs. Gold Plan
Exclusions
- Cannot be used for purchase of dental retail products.
- Total payment amount is due at the time of service. If full payment is not received at time of service, fee reduction will be void.
- Cannot be combined with any other dental insurance or discount.
- Discount not applied to Zoom! QuickPro In-House Whitening.
- Payment is due at the start of each membership year.
- Courtesy fee reduction or included dental services are non-transferable.
- Fee Reduction does not apply to Morrison Migraine Center.
- Members must pay the application fee and membership fee prior to receiving a membership card.
